First get a car that is decent. Then go up to a hooker on the sidewalk and just stop your car by them. Then they will bend over to talk to you and then they will get in your car just pull the car to a semi-deserted place and have fun.

Once you have played a level you can skip the movie cuts, but it is not possible to skip the animated stuff.

Yes. Each club is of a different height but for the driver it should come up to the bellybutton of the person using the club, or just above so you can choke up on it.
