It is a tired cliche among fans that his name or pseudonym is not Who.

As the question states, why else is the program called 'Doctor Who?'

Its true that he is seldom referred to as 'Doctor Who', except in The War Machines (which is a clear indication that it is his name or pseudonym). Neither is he ever called Theta Sigma, except by Drax in The Armageddon Factor, but that is definitely a name the Doctor has used.

He called himself 'Dr Von Wer' in The Highlanders, which means Who in German. He has signed his name as Dr. W before.

Who might not be his real name, but it is clearly a name that he has been called or has used.
