I am not entirely sure whether you are legally obliged to provide ANY information about race - only about nationality. Many ethnic monitoring surveys include a sentence to say that completion is voluntary.
Ethnic monitoring surveys are designed to monitor the ethicity of a company, or a service to ensure that a mix of people are employed, or receive the services. It does not form an integral part of a job application, or a request for a service.
For example, I never complete the ethnic monitoring surveys at my local hospital, as i would rather receive the treatment that I need rather than the treatment that is appropriate for someone of my ethnic background.
A small Kentucky church has chosen to ban marriages and even some worship services for interracial couples. Is this legal?

tks for your answer, let me clarify my question and hopefully you can still help. can i switch back and forth? pick black on one form, asin on another, white on the next or am i legally obligated to stay consistant with my selection?
