There can be many and overlapping causes for either answer, including, but not limited to:
-poor teaching
-illness
-disability
-unsupportive home environment
-homelessness
-material isn't taught in the student's language
-student is bored
-student doesn't think academics are important
-student has much more serious/critical problems such as family illness, bullying, teen pregnancy
-student is lazy

Gotcher back, there, Lyn. That was a great answer. It can only be 1 or 2 or both. And, if it's 2, it's back to 1 anyhow. In my oppinion it comes back to the teacher 99 44/100 percent! All of your reasons are sound but a really caring teacher could overcome most if not all of them. Great answer!
