Lots and lots of cardio. Running is the most effective, but be very discriminating with the shoes you wear to run in. Running really puts punishment on your joints. An elliptical is great, and burns a good amount.

It would depend on your level of commitment. If you're really committed (most people aren't not matter how much they like to say they are), it could (should) safely take you two months. It sounds like a long time, but it passes quickly.
On average, it can take a good four months, which also IS NOT so long. You just have to treat it like a war or something.
