What was the highest speed ever attained by the USS Enterprise, and which episode did this occur?

    There was this one episode, I can't remember the title, but an alien woman came on board the Enterprise undetected, and fused something or other in the warp drive. The ship accelerated past Warp factor 14.1. Scotty had to crawl through a Jeffries Tube with a magnetic probe to "unfuse" whatever it was before the hull came apart from the stress. As far as I know, that was the highest speed ever reached by the Enterprise.
