Absolutely. Does it have to? Nope. The usual types of injury are torn tissue, strained muscles, and infection, and you can prevent those by applying some basic care and attention.

First, do not have anal sex without using a lubricant. Ever. Unlike vaginas and mouths, anuses are not self-lubricating, which means it's very easy for the tissue inside to tear if it's not kept wet enough. That's both painful and dangerous. Don't "make do" with something not intended as lube, and don't skimp on it. (Remember, water-based lube only if you're using a condom . . . which is also a good idea for hygenic reasons.)

Second, take it slow, especially when the anus in question is not used to being penetrated. There are involuntary muscles inside which have to relax in order for anal sex to not be painful, and that can take time. Be patient, and more importantly, be willing to stop if it's not going well.

    Healthy fun to you.
