What does 'my bad' mean, and do Americans use it a lot in both written text and conversations?

    "My bad" is a very informal way of saying "I was mistaken" or "It was my fault" or (to use the Latin phrase) "mea culpa". As joeyfg mentioned, it was popularized somewhat by the movie "Clueless". In my experience, it is still being used but I would not call it very common. It seems to found mostly in conversation, among friends, and among the younger generations (which are not surprising given its informality). It also feels (according to my native U.S. speaker intuitions) to be an aspect of "valley girl" speech (though not limited to it). That may be due to its use in the movie "Clueless" or perhaps that is why it was used in that movie.
