Help answer this question below.

What causes a person to be paranoid, scared, and having trouble differentiating real events form things you are afraid of? ( I already thought of anxiety)?

    Well, if it is severe enough that your friend is really unable to differentiate reality from fantasy I would say paranoid schizophrenia. But if they are just generally scared of unlikely events, I would say maybe just a generalised phobia.
