Does the icy hot patch give off a icy & hot feeling or how does it work..?

At first it is freezing cold and makes you scream which makes the person putting it on your back (assuming it's going on your back) laugh hysterically. Then after a few minutes it gets warm, but it's a deep soothing warm. I love these things! Except when they are first put on, and when I forget they are there and find them when I'm taking a shower and it's all wet and icky.
